Australian Shepherd Comprehensive Breed Health Survey

A link to the survey is below, but before you begin there are a few things that will assist you in filling out the form

  • You may enter as many dogs as you want, but only one at a time.
  • Dogs entered must have been born from 1990 - 2005.
  • Data from both living and deceased dogs is encouraged.
  • Have all your dog's health and breeding records available for quick reference
  • If you have all information on your dog handy for reference, the survey should not take over an hour to complete.
  • If you must leave the computer before you complete the survey, do not use the continue button at the bottom of the page unless the page is complete. If you have created a user profile and leave the survey before finishing, you will be able to go to the page AFTER the last page you completed. The information on an un-completed page, however, will not be remembered.
  • IMPORTANT NOTICE ABOUT PASSWORDS: If you elect to set up a user ID for the survey and your computer has password manager software we strongly recommend that you have it remember your user ID and password. The passwords supplied by the survey software are encrypted and not recoverable by Elements Software Engineering. If you cannot remember your password and the hint you supplied is insufficient, request a temporary password. Once you have received that you can reset the password to something you will remember.
  • If you must leave the survey before you complete it, you may check back in later with your log-in but you will have to start at the page after the one on which you last made entries. IMPORTANT: This is only an option if you have created a user profile. If you have not created a user profile and leave the survey before completing it, the dog's information can not be made complete.
